Passengers Requesting Special Assistance (PNAE)
United strives to offer safe, reliable and accessible transportation for all customers. For more information, visit the Special Travel Needs page. If special assistance is needed during your travel, please call the United Customer Contact Center in Brazil at least 72 hours prior to your flight departure if you are traveling with a companion or at least 48 hours prior to departure if you do not require a companion. Certain customers must also complete and submit a Medical Information Form (MEDIF) to United’s designated medical professionals by email (medif.brasil@united.com) at least 72 hours prior to flight departure.
The form must be completed for customers who:
- Need to use oxygen or other medical equipment; or
- Present health conditions that may pose a risk to themselves or other passengers, or may need special medical attention during a flight
The transportation of customers in such situations is subject to prior authorization by United’s designated medical professionals, based on information about their physical and mental conditions, provided by the customer or by his physician in the MEDIF.
Companion
To ensure the safety of all of our passengers, the following customers must travel with a companion:
- Customers who, because of a mental or intellectual disability, are unable to comprehend or respond appropriately to flight safety instructions; or
- Customers who cannot meet their own physiological needs without assistance
These customers (or their legal representative) must complete the MEDIF at least 72 hours prior to their flight. A United representative will confirm the need for assistance. Companions who have a ticket issued in Brazil and are departing from an airport in Brazil will receive an 80% discount on airfare. Companions must be able to take care of the personal and medical needs of the passenger.

How to complete the forms:
- Print the four pages.
- Read, sign and complete the forms. Some forms will need to be completed by the passenger’s attending physician.

- Completed and signed by the passenger or legal representative.

- Page 2 completed and signed by the attending physician.
- Page 3 signed by the passenger or legal representative.
- The physician must include their CRM number, email, name of medical clinic and mobile phone number

How to submit the forms:
- After correctly and legibly completing the forms, scan pages 1 - 3.
- Send the scanned files to medif.brasil@united.com with the email subject line: “Completed MEDIF – [TRAVELER’S CONFIRMATION NUMBER]”.
After you’ve submitted the form, a United representative will call you to provide any necessary information.
